Funding (up to $2500) may be available for UBC MPH students during their practicum. Eligibility is determined on an individual, needs-based assessment. Please return this form once completed to Kim Mantle for approval. Once students have been approved for practicum funding, they are appointed as Graduate Research Assistants. If you have never been employed by UBC (and thus do not have an employee ID), please also download and complete the Direct Deposit form.

Criminal Records Check

UBC is subject to the requirements of the Criminal Records Review Act, which means that all students who are enrolled in programs that include a practicum component involving work with children or vulnerable adults will have to undergo a criminal record check before they will be permitted to register in the practicum. If you are found to present a risk of physical or sexual abuse to children, or physical, sexual, or financial abuse to vulnerable adults, as a result of the CRRA Check, you will not be permitted to register in the practicum.  If the practicum is required for your program, this means that you may not be able to complete the program and you may not graduate. Therefore you should consider this requirement carefully before applying to, or continuing in, this program.

If students are considering a practicum where they will be in direct contact children or vulnerable adults, please contact kim.mantle@ubc.ca as soon as possible.

Practicum Deliverables

MPH students are required to submit at the end of their practicum:

1. Two completed final assessments (see forms #6 & #7)

2. Practicum output(s) as indicated in the Learning Contract (report, presentation, publication, etc.)

3. Prepare a poster presentation for the annual September "MPH Meet & Greet and Practicum Showcase"

Additional Practicum Funding Announcement:

The Women’s Health Research Institute in Vancouver will support an MPH student undertaking a practicum in maternal child health with an honorarium of $2,000 in Summer 2011.  To apply students should forward their practicum learning agreement to kim.mantle@ubc.ca.  The placement can be at any site so long as it focuses on maternal child health.
